  • Enter your information in the fields as required. Enter your 15 most predominant ingredients, in order of predominance.

    Be as specific as possible in completing the source and location of each product (state the manufacturer and their location, not simply the retailer). You may combine ingredients where applicable (i.e. the same supplier for fruit or spices), providing they all share the same traits (i.e. supplier, location, organic or not). It is understood that there may be seasonal changes to your Food Vendors Statement of Ingredients. You may make changes to your Statement of Ingredients using masking tape and a marker (for example, changing the information of strawberries to organic and a stating a local supplier when in season). For permanent and numerous changes, you may make changes to your Statement of Ingredients by resubmitting this form.

    Moss Street Market food vendors may be asked by the market to substantiate information on their Statement of Ingredients, therefore retain all of your invoices/receipts for the season for all items listed on your Statement of Ingredients as local and/or organic.
